Guest Stay Registration Packet

This example will use the PDF received from Idaho to simulate.

Three simple steps:

1. Enter your Stay Guest ID # and Patient Last Name and click on Get My Registration Packet

2. Review Your Packet and validate the information is accurate.  Adjust if needed and Save.

3. Sign & Submit.

 

On success your will be automatically redirected to a confirmation page with more instructions
